I was a Consultant Vascular, Endo-vascular surgeon the Pennine Acute Hospitals NHS trust, one of the largest NHS trusts in the United Kingdom (www.pat.nhs.uk). The trust contains the regional vascular surgical unit for the north of the greater Manchester region, covering a population of around 1.3 million people. The unit is one of the busiest vascular units in the UK. The unit provides the full range of vascular services and supports the departments of Cardiology, Nephrology, Endocrinology including diabetes and Trauma. The unit performed in excesses of 160 carotid endarterectomies and in excess of 120 aneurysm repairs in the last 12 months.The unit performs in excess of 300 peripheral vascular interventions, both open and endovascular, per year.

I was the Clinical Director for Vascular and endovascular services. The unit is staffed by 6 consultant vascular surgeons (including myself), 6 specialist registrars, 3 core trainees and 4 foundation doctors. The unit is well supported by a comprehensive vascular laboratory, 24/7 diagnostic and interventional radiological facilities, including multislice CT scanners, MR scanners and two state-of-the-art modern angiography and interventional radiology suites. A Hybrid Operating room with a state-of-the-art GE Discovery 740, imaging facility is fully operational. There is a 40-bedded vascular unit run by trained nurses as well as a vascular specialist nurse.

My clinical working week involved one full day a week operating on major vascular cases, one half day a week operating on day-case patients, two half days a week in outpatient clinics and one to two private operating lists a week. One half day a week is set aside for the multidisciplinary meeting. I also teach medical students, house officers and vascular nurse specialists. I train the Specialist Registrars and the Senior House Officers in operating theaters, wards and outpatient clinics. In addition to once a week teaching ward rounds I perform 3 business ward rounds a week.

My surgical practice is 100% Vascular Surgery. I have a special interest in aneurysm repairs (including EVAR) and carotid endarterectomies and peripheral vascular disease including diabetic vascular disease. The common operations I perform are:

In-patient

• Open aortic aneurysm repairs (elective & ruptured)
• Endo-vascular aortic aneurysm repairs- EVAR & TEVAR
• Carotid endarterectomy
• Aorto-bifemoral grafts for aortic/iliac occlusions
• Femoro-popliteal and distal bypass grafts
• Thoracic outlet decompression (1^st cervical rib excision)
• Mesenteric bypass
• Carotid subclavian bypass
• Thoracic sympathectomy
• Treatment of diabetic feet (sympathectomy, amputations etc)
• Trans femoral and trans-brachial Embolectomy
• Iliac angioplasties and stenting
• Infra-inguinal angioplasty and stenting
• Remote endarterectomies
• Thrombolysis for acute peripheral arterial and graft occlusion
• Thrombolysis for venous system including acute DVT
• IVC filter placements

Out-patient (Day case)

• Varicose vein treatment (surgery & radio frequency ablation & endovenous laser therapy)
• Botox for axillary and palmar hyperhidrosis
• Creation of A-V access for dialysis

I have an emergency on-call commitment of one in six. We provide vascular emergency services to the population in the northern half of the greater Manchester region. The common emergencies I deal with are:

• Ruptured aortic aneurysms
• Acutely ischemic limbs
• Crescendo TIAs
• Vascular trauma
• Gun shot and knife injuries to the vascular system

Cooperating with the radiologists and stroke physicians, I also provide a lower limb and carotid (post stroke) thrombolysis service.

Prior to my present position, I took up a sabbatical and established the vascular & endovascular services at the Kokilaben Dhirubhai Ambani Hospital (part of Reliance ADA group) a flagship multi-specialty tertiary referral center in Mumbai, India.

I also have practicing privileges in two private hospitals namely, the BMI Priory Hospital in Edgbaston and the BMI Highfield in Rochdale.

My results for index procedures for aneurysm repairs and carotid interventions are one of the best in the United Kingdom. This is available for public access on the NHS choices website. You will note that my results are excellent.

• I am the Clinical Director for Vascular & Endovascular therapies at Pennine Acute, with managerial responsibilities for the entire Division of Vascular Surgery.
• I established a modern Vascular & Endovascular surgical unit in a new tertiary referral center in Mumbai, India.

• Negotiated with Lombard medical and secured the loan of a mobile C-arm image intensifier worth £100,000 for use in the theater for vascular imaging

• Along with colleagues and stroke physicians, successfully changed practice so as to implement the National Stroke Strategy in the West Midlands.
• Pursued the objective of training a person to undertake vascular duplex scans (person successfully trained and in post)
• Worked towards achieving closer links with City Hospital and Sandwell Hospital in terms of improving interventional radiology support as well as joint multi-disciplinary team meetings
• Performed the first radio-frequency ablation procedure for varicose veins at the Manor Hospital
• Developed information leaflets for all vascular surgical conditions treated
• International proctor for the Lombard Aorfix endovascular stent graft
• United Kingdom proctor for the Endologix AFX stent graft
• Was awarded 2 Clinical Excellence Awards after just one year of service.

• I run the Pennine endovascular aneurysm sizing & deployment course, twice a year. This is a simulator-based course.

• When I joined the vascular unit at the Royal Liverpool University Hospital in 1996, EVAR was in its infancy. The units, led by Mr. P. Harris, were pioneers in this new form of AAA therapy. I had the opportunity to observe, from close quarters, these pioneering efforts. I was privileged to be able to be a part of the learning process. At that point, the old Vanguard devices were being used and I got to see many graft migrations, stent kinks & occlusions and strut fractures and innovative ways of tackling them. The year at the Royal Liverpool helped establish my interest in EVAR.

  My tenure at the Manchester Royal Infirmary (MRI) coincided with the onset of the EVAR trials for which the MRI was a participating center. As a clinical fellow in vascular surgery, I was actively involved in the selection of patients, randomization, performing EVAR and the collection and transmission of data. I attended several meetings of the EVAR trial centers on behalf of Prof. M. G. Walker. The devices used at the MRI were the Cook Zenith and the Medtronic talent.

  During my year at the South Manchester University hospital, working for Prof. C. McCollum, I had the opportunity to work closely with Dr. Ray Ashleigh (consultant interventional radiologist) and gained a vast amount of experience in working up patients (sizing, selection of graft etc.) and independently performing graft deployments. Medtronic, Talent device was the device of choice at South Manchester at that time. I vastly improved my knowledge and practical skills in interventional radiology during this post.

  After being appointed as consultant surgeon at Walsall, I single-handedly, established the EVAR service. This was a major challenge to do in a cash strapped DGH with colleagues who were initially skeptical. I started using the Lombard Medical (Aorfix) device and the Endologix AFX device. I gained considerable experience with these devices. I have trained several units in the UK and abroad in the use of these devices. I have proctored several vascular colleagues in India, helped them set up an Endo-vascular aneurysm repair service. This included EVAR as well. After having performed the first endovascular repair for a ruptured aneurysm at Pennine, I was able to establish it as part of our routine practice.
